Clinical Effects of Captopril in Essential Hypertension



Abstract
To evaluate antihypertensive effect, side effects and metabolic changes of captopril, capoten¢ç alone, 50-100mg / day, or combined with dihydrochlorothiazide, 25mg / day, was administered for 10 weeks in 15 hypertensives without renal impairment. In 12 mild to moderate hypertensives (initial BP, 180+16 / 102¡¾7mrnHg), mean systolic BP and diastolic BP reduced 47mmHg and 9mmHg respectively with captopril monotherapy. In 7_ of 12(58%), BP decreased to below 140 / 90mmHg. In
3 patients of severe hypertension (initial BP, 212¡¾13/12 3¡¾5inmHg), mean systolic BP and diastolic BP reduced 65mmHg and 13mmHg respectively with combined therapy with dihydrochlorothiazide. There was no significant side effects except one case of intermittent cough and two cases of hunger pain and there was no significant impairment in quality of life. In cases with
captopril montherapy, serum total c-lolesterol, HDL-cholesterol, triglyceride and creatinine didn¢¥t change significantly but K+ increased slightly (P=0.05).
These results suggest that captopril is an effective and safe antihypertensive drug and can be used as a first line drug for the treatment of essential hypertension.
